EMPLOYEE #1 WAS SENT INTO A GRAIN ELEVATOR STORAGE TANK TO FREE A CLOGGED DRAG C ONVEYOR. HE WAS NOT PROVIDED WITH A SAFETY HARNESS. HE WAS PULLED INTO THE FLOWI NG GRAIN, BURIED AND SUFFOCATED.
