Investigation abstract
AFTER LUNCH WORKERS WERE PROCEEDING TO THEIR ASSIGNED WORK AREAS. EMPLOYEE #1, A SAWYER, WENT TO THE DOWNHILL SIDE OF THE D-6 SKIDDER. THE D-6 ROLLED OUT A BOUL DER APPROXIMATELY ONE CUBIC YARD IN SIZE (WEIGHING ABOUT 3,000 POUNDS) WHICH ROL LED DOWN THE STEEP GRADE (ABOUT 45%), STRIKING EMPLOYEE #1 AND CRUSHING HIS HEAD .
