Employee killed when caught by overhead auger in storage bin

Investigation abstract

Employee #1 climbed into the top of a two-story shavings bin to retrieve a hard hat that had been sucked through a ventilation pipe. An overhead auger was autom atically activated, presumably by Employee #1's movement inside the bin. His hea d became caught by the auger, and he was killed.
