EMPLOYEE #1 WAS STRUCK BY A FLY-AWAY JIB (BOOM EXTENSION) WHICH WAS NOT SECURED BEFORE OPERATION. THE EMPLOYEE WAS HIT IN THE HEAD BY THE JIB. HE WAS STANDING B ELOW IT WHEN IT CAME LOOSE.
