Employee killed in fall from knuckle boom
Investigation abstract
Employee #1 was using a knuckle boom to load a log truck when he struck the truc k's mirror. After completing the lift, Employee #1 leaned over to evaluate the d amage to the mirror and toppled out of his on the knuckle boom. He fell 12 1/2 f t, striking the ground head first. Employee #1 was killed.
