Investigation abstract

EMPLOYEE #1 WAS OPERATING A FURNACE CHARGING MACHINE. WHILE HE WAS CHARGING THE FURNACE THROUGH THE FRONT CHARGING DOOR, A BALL OF FLAMES SHOT OUT OF THE DOOR A ND ENGULFED THE MACHINE'S CAB BRIEFLY. EMPLOYEE #1 SUFFERED SECOND AND THIRD DEG REE BURNS OVER MOST OF HIS BODY. HE WAS KILLED.
