CONSTRUCTION EMPLOYEE KILLED IN FALL THROUGH ROOF
Investigation abstract
EMPLOYEE #1, A WHEELBARROW OPERATOR, WAS HAULING SCRAP MATERIAL FROM A SECTION O F ROOF BEING REPLACED TO A DEBRIS CHUTE LOCATED AT THE SIDE OF THE BUILDING. AS HE RETURNED FROM DUMPING A LOAD HE STEPPED ON A PRECUT 32 BY 42 INCH PIECE OF GY PSUM THAT BROKE UNDER HIS WEIGHT. HE FELL 25 FEET, 9 INCHES TO THE CONCRETE FLOO R BELOW AND WAS KILLED. TWO OTHER EMPLOYEES WHO WERE WORKING WITH EMPLOYEE #1 ST ATED THAT HE TOOK THE WRONG PATH BACK TO THE WORK AREA.
