LEG CRUSHED BETWEEN LOG CARRIAGE AND SIDE RAIL

Investigation abstract

EMPLOYEE #1 STEPPED INTO THE PATH OF A RETURNING LOG CARRIAGE. THE CARRIAGE CAUG HT HIS LEG, CRUSHING IT AGAINST THE SIDE RAIL. THE AREA WAS POSTED WITH WARNING SIGNS, AND WAS PROTECTED BY GUARD RAILS. THE EMPLOYEE WAS TAKING A SHORT CUT ORD INARILY USED FOR TRANSPORTING THE CIRCULAR SAWS TO AND FROM THE SAW SHARPENING R OOM (WHEN THE MILL IS SHUT DOWN). EMPLOYEE #1 (A MILLWRIGHT) WAS ACCUSTOMED TO U SING THIS ROUTE WHEN THE MILL WAS SHUT DOWN. HE USED THE ROUTE WHEN HE WAS SERVI CING THE MACHINERY.
