EMPLOYEE FELL WHEN UNSECURED PLANK SLIPPED OFF ITS SUPPORT

Investigation abstract

AT APPROXIMATELY 2 PM ON DECEMBER 5, 1986, EMPLOYEE #1, A POWER PLANT CONTROLS O PERATOR TRAINEE, WAS STANDING ON AN ELEVATED 2 INCH BY 12 INCH PLANK, SHAKING A 14 INCH PIPE TO CLEAR A BLOCKAGE. THE PLANK SLIPPED OFF ONE OF ITS SUPPORTS, ALL OWING EMPLOYEE #1 TO FALL APPROXIMATELY 8 FEET, 4 INCHES, AND STRIKE HIS HEAD ON THE TOP OF A CONCRETE RETAINING WALL. HE THEN FELL AN ADDITIONAL 5 FEET, 7 INCH ES, STRIKING THE BACK OF HIS HEAD ON THE CONCRETE FLOOR. THE EMPLOYEE DIED ON DE CEMBER 6, 1986, AT APPROXMATELY 8:20 PM, AT THE WAHIAWA GENERAL HOSPITAL.
