Event description

CAUGHT BETWEEN TWO ROTATING SHAFTS

Investigation abstract

ON AUGUST 6, 1984, AT APPROXIMATELY 11:30 P.M., EMPLOYEE #1 WAS ON TOP OF A BANB URY MOTOR SHAFT COVER, CLEANING IT. THE COVER HAD A 19" X 15 " OIL ACCESS HOLE O N IT. THE HOLE HAD A HINGED COVER, BUT THE HINGE HAD APPARENTLY BROKEN OFF. THIS ALLOWED THE COVER TO SLIP OFF THE HOLE, LEAVING THE ROTATING SHAFTS LOCATED BEL OW EXPOSED. EMPLOYEE #1'S RIGHT FOOT SLIPPED THROUGH THE HOLE AND WAS CAUGHT BET WEEN THE SHAFTS. HIS LEG WAS THEN PULLED IN UP TO HIS THIGH. HE WAS FREED WITHIN 30 MINUTES AND TRANSPORTED TO THE BAPTIST HOSPITAL, BUT DIED ON AUGUST 12, 1984 , AT APPROXIMATELY 5 P.M.
