TRUCK CARGO FELL OFF TRUCK BED, KILLING EMPLOYEE

Investigation abstract

AT APPROXIMATELY 1 PM ON OCTOBER 2, 1986, EMPLOYEE #1 WAS TOLD TO DIG OUT THE LE FT REAR DUALS OF A 10 WHEEL HAUL TRUCK WHICH HAD BECOME STUCK IN A POTATO FIELD. AS HE WAS DIGGING, THE BUNKER BED, LOADED WITH FRESHLY DUG POTATOES, SEPARATED FROM THE MOUNTING BRACKET WHICH HELD THE BED TO THE TRUCK FRAME. THE LOADED BED TIPPED AND FELL OFF THE TRUCK, STRIKING EMPLOYEE #1 IN THE HEAD, KILLING HIM.
