EMPLOYEE KILLED IN FALL FROM BOX BEING LOWERED BY A CRANE
Investigation abstract
EMPLOYEE #1 WAS INVOLVED IN THE LOADING OF CONTAINERS ABOARD A SHIP. AT THE TIME OF THE ACCIDENT HE WAS RIDING IN A BOX SUSPENDED FROM THE CRANE. AS THE BOX WAS BEING LOWERED TO THE SHIP'S DECK HE FELL OUT FROM THE BOX'S OPEN DOORWAY ABOUT 16 FEET TO THE DECK. HE LANDED ON HIS HEAD AND WAS KILLED.
