FALL FROM SCAFFOLD

Investigation abstract

EMPLOYEE #1 WAS WORKING ON THE TOP LEVEL OF THE SCAFFOLD ON THE DAY OF THE ACCID ENT (NOVEMBER 4, 1984, 1:40 PM). HE HAD BEEN WORKING AS A FIRE WATCH, MOVING FRO M ONE SIDE OF THE RUDDER SCAFFOLDING TO THE OTHER. IMMEDIATELY PRIOR TO THE FALL , EMPLOYEE #1 HAD BEEN STANDING ON THE PORT SIDE OF THE RUDDER SCAFFOLDING, WHIC H WAS ABOUT 30 FEET HIGH. THE SCAFFOLDING WAS OLD AND HAD NO MANUFACTURER NAMES OR SERIAL NUMBERS. ONE EMPLOYEE, WHO BELIVES HE SAW THE FATAL EVENT, SAID THAT E MPLOYEE #1 HAD BEEN LEANING AGAINST THE SIDE RAILING WHEN IT GAVE WAY. THE EMPLO YEE FELL TO THE DECKING AND WAS KILLED.
