Event description
Employee killed when struck by machine parts under pressure
Investigation abstract
Employee #1 was inflating a tire on the air-feed shaft of a Cincinnati buffing m achine, model 50A, serial #100488, when the threaded portion of the shaft failed . The bolt sleeve and retaining plate were propelled with great force due to the sudden pressure release. Some or all of the pieces struck Employee #1 in the he ad, causing a serious concussion. He was killed.
