EMPLOYEE CRUSHED BETWEEN TRANSFORMER AND CABLE
Investigation abstract
EMPLOYEE #1 WAS WORKING ON A LADDER AT THE FRONT OF A 15 FOOT HIGH TRANSFORMER W HICH WAS BEING TRANSPORTED ON A LOWBED TRAILER. HIS JOB WAS TO PUSH LOW HANGING CABLES UPWARD SO THEY WOULD NOT HANG UP ON THE TRANSFORMER AS THE TRAILER MOVED FORWARD. APPARENTLY ONE OR MORE CABLES WERE NOT PUSHED OUT OF THE WAY SOON ENOUG H, AS THE EMPLOYEE WAS CAUGHT BETWEEN THE CABLE AND THE TRANSFORMER. HE WAS CRUS HED TO DEATH. THE TRUCK DRIVER OR A HELPER SHOULD HAVE BEEN MONITORING THE WORK OF EMPLOYEE #1, AND THE TRUCK STOPPED IF ANY DIFFICULTY DEVELOPED.
