Event description

Employee injured in fall from ladder, later dies

Investigation abstract

On June 2, 1986, Employee #1, a laborer, was sent by his foreman to get shovels for backfilling a retaining wall from the far (south) side of a 6 ft 8 in. concr ete wall. A defective 12 ft ladder was used as a short cut to get to the other s ide of the wall. While climbing the ladder, Employee #1 apparently fell to the c oncrete floor on the near (north) side of the wall. He suffered a fractured skul l, from which he died nine days later. There were no witnesses to the accident.
