AT APPROXIMATELY 4 PM ON DECEMBER 19, 1986, EMPLOYEE #1, A CONSTRUCTION WORKER, FELL ABOUT 10 FEET FROM A SCAFFOLD TO A CONCRETE FLOOR. HE DIED THE NEXT DAY FRO M THE INJURIES HE SUSTAINED.
