FALL FROM ELEVATED WOOD PALLET
Investigation abstract
A THREE MAN WORK CREW WAS PLACING 20 FOOT LONG STEEL RACKS IN A VERTICAL POSITIO N FOR STORAGE IN A WAREHOUSE. EMPLOYEE #1 WAS RAISED TO A 13 FOOT HEIGHT 0N A WO OD PALLET BY A POWERED INDUSTRIAL TRUCK. THIS WAS DONE TO FACILITATE THE REMOVAL OF A WOOD PALLET THAT WAS BANDED TO THE STEEL RACKS. WHILE REMOVING THE PALLET EMPLOYEE #1 LOST HIS BALANCE AND FELL, LANDING ON HIS HEAD.
