Investigation abstract

AT APPROXIMATELY 9:30 A.M. ON JUNE 17, 1988, EMPLOYEE #1 WAS REMOVING A SKYLIGHT FROM THE ROOF OF BLACK RIVER ELEMENTARY SCHOOL WHEN HE ACCIDENTALLY FELL THROUG H THE SKYLIGHT OPENING TO THE FLOOR, A DISTANCE OF APPROXIMATELY 28 FEET. EMPLOY EE #1 DIED AS A RESULT OF THIS FALL.
