Event description

Employee crushes thumb in Brake Press, leading to partial am

Investigation abstract

At 10:30 a.m. on May 5, 2021, Employee #1 was completing bends on an Accupress Brake Press. After completing one bend, he reset the part and performed a second bend. During the second bend, the employee's left thumb was crushed between the Brake Press die and the part. The end of his thumb was pinched off, with the injury affecting only the flesh. An emergency room doctor attempted to reattach the flesh, but it could not be successfully done. Employee #1 was hospitalized for treatment.
