Employee amputates left pinky finger on tape machine pneumat
Investigation abstract
At 4:25 a.m. on May 7, 2021, Employee #1 knelt beside a maintenance employee to observe repairs on a tape machine on Line 4. Upon standing, the employee grabbed the north mast of the machine, and his left pinky finger entered a hole located on the east side of the mast. While the tape machine was running, an operator sent a box through, causing the floating upper head and pneumatic internal column to move up to fit the box size and tape it. After the box passed, the internal column returned down and contacted the employee's pinky finger inside the hole, amputating the fingertip. The employee was treated without hospitalization.
