Event description

Employee is killed when crushed on guardrails

Investigation abstract

On June 7, 2021, an employee was working from a rod basket performing gas and and sucker rods hanging in the derrick. The employee was crushed when he was pinned to the guardrails and rod basket. The employee suffered injuries to his abdomen and asphyxiation and was killed. oil well servicing operations removing sucker rods and a subsurface pump. The employee was hanging the sections of three sucker rods onto a derrick. During the removal of the rods the employee attached a transfer cable and swivel to a string of rods and released the elevators attached to blocks before the bottom section was supported by the bottom elevators. This caused the entire string weight to be placed onto the transfer cable (a 5/16 wire rope) resulting in a rope break. When the rope broke, a cable clamp attached to the rope snagged the employee's SLR cable, pulling the employee down onto the top of the guardrail
