Employee is struck by pallet jack while parking and sustains
Investigation abstract
At 6:30 p.m. on May 27, 2021, Employee #1 was parking a Raymond pallet jack (Model 8410) when she stepped off the equipment before it had come to a complete stop. The pallet jack continued moving and struck her leg, pinning it between the jack and a barrier. She sustained leg injuries and was hospitalized, treated, and released after two nights.
