Employee partially amputates finger while adjusting form mac

Investigation abstract

At 6:00 a.m. on May 18, 2021, an employee working as a machine operator for a snack food manufacturer was operating a horizontal form, fill and seal machine. The employee did not deenergized the machine before climbing the ladder and attempting to adjust the brush mechanism. The employee caught her left hand in the machinery. She was hospitalized to treat a partially amputated middle finger and an injured index finger.
