Event description

Employee's fingertip is amputated while aligning rack holes

Investigation abstract

At 7:30 p.m. on June 13, 2021, two employees were in the process of setting up racks. While attempting to align the holes for pin installation, Employee #1 inserted his finger into one of the holes to determine the needed adjustment. At that moment, the other employee raised the rack, pinching Employee #1's finger between the top and bottom tiers of the rack structure. The incident resulted in a fingertip amputation.
