Event description
Employee fractures both ankles after forklift strikes pallet
Investigation abstract
At 9:49 a.m. on June 11, 2021, Employee #1, a temporary worker employed by at Arrowhead Regional Medical Center. Insufficient training and lack of verification of forklift operation competency contributed to the incident. A serious citation was issued under T8CCR �3668(a) for Powered Industrial Truck Operator Training. Workforce Enterprises WFE Inc., was performing general cleaning and moving items in a distribution warehouse operated by a host employer. While operating a Raymond stand-up counterbalanced lift truck (Model #425-C40TT), the employee was instructed to move a broken rack component. While driving the forklift in reverse between Amazon Racks B and C, the employee failed to see a stack of pallets located behind another pallet. The forklift struck the pallets, causing them to shatter. Fragments entered the cab and struck the employee's legs, resulting in fractures to both ankles. The employee was hospitalized and treated
