Employee suffers fingertip amputation while bending metal pa
Investigation abstract
At 12:30 p.m. on June 14, 2021, Employee #1 was bending metal parts on the LVD #181 press brake. After completing the first bend, the employee repositioned the part for a second bend and held onto it as the machine engaged. During the bending operation, the employee's right thumb was pinched between the part and the front side of the press brake, resulting in a partial fingertip amputation. The metal part was approximately 1.5 inches wide, and additional safeguarding methods, such as using a tool to maintain hand clearance from the point of operation, were not in place at the time. The incident occurred indoors and was not weather-related. The employee was treated without hospitalization.
