Employee amputates finger when caught in hydraulic valve

Investigation abstract

At 2:20 p.m. on June 17, 2021, Employee #1 was installing a hydraulic valve. While checking if the valve was functioning properly, the employee placed a finger inside the valve. The valve was energized and closed on the employee's finger, resulting in an avulsion amputation. The employee was hospitalized for treatment.
