Event description

Employee suffers fingertip amputation while operating router

Investigation abstract

At 8:15 a.m. on June 14, 2021, Employee #1 was trimming dashboard parts using the DMS 5-axis router #1. While operating the machine, the employee used his left hand to hold down a vibrating part, which came into contact with the trimming blade. The employee suffered an amputation of the tip of his left index finger. The DMS 5-axis router #1 was not properly guarded at the time of the incident. The employee was admitted to the hospital for treatment.
