Employee?s fingertip is lacerated while hand drilling on lad

Investigation abstract

At 1:00 p.m. on May 26, 2021, Employee #1, a high-level mechanic, was performing maintenance on regenerative thermal oxidizer (RTO) #2 at a spirit plant. While hand drilling a steel shaft and block on a ladder without scaffolding or a temporary platform, the employee slipped and the drill struck the fingertip. The employee sustained a laceration with loss of fingernail and part of the distal tip, but no bone loss. The employee was treated without hospitalization.
