Event description

Employee fractures lower arm in press dies, requires surgery

Investigation abstract

At 1:15 p.m. on June 29, 2021, an employee working as a pressing machine operator for an HVAC and commercial refrigeration equipment manufacturer was operating an Oak Fin Model FP-3B Progressive Die Press. The employee noticed the aluminum sheet had become stuck to the top die. The employee placed his hand between the top and bottom die to dislodge the metal sheet. The press cycled while the employee had his hand inside the point of operation. The employee was hospitalized to treat bone fractures to his arm, which required surgery.
