Investigation abstract

At 3:00 p.m. on June 24, 2021, an employee was using a rolling machine to form a piece of 12-gauge steel when the employee's right hand was pulled into the rollers. The employee sustained amputations of portions of all fingers on the right hand. The employee was hospitalized for treatment.
