Event description

Employee is killed when car falls off jack

Investigation abstract

At 5:00 p.m. on May 12, 2021, an employee, working part-time in a tire shop, was under a customer's car changing the oil. The car was lifted and supported by a jack but there were no jack stands. The jack failed and the car fell onto the employee resulting in crushing injuries to his head and abdomen and severe bleeding. The employee died from his injuries.
