Employee incurs severe lacerations after being hit with 22,0
Investigation abstract
At approximately 3:00 p.m. on June 3, 2020, an employee along with coworkers was disassembling a flange test adapter on a customer's equipment after it had undergone 22,000psi hydrostatic pressure testing. The employee was loosening the bolts holding the test flange to the equipment when pressurized water escaped and hit him in the upper legs. The employee suffered severe lacerations to both thighs and was hospitalized.
