Employee amputates finger tip when caught in pulverizer
Investigation abstract
At approximately 12:05 p.m. on July 8, 2021, an employee was instructed to go to lunch by a supervisor and to stop work. The employee made an adjustment to the pulverizer that the employee was previously working on after being informed of the need to take a lunch. Employee removed a sock on the pulverizer and stuck their hand into an opening and contacted the blades while the machine was still running. The employee amputated the left index and middle fingertip. The employee was hospitalized for treatment.
