Event description
Employee is struck by slipping die, sustains head laceration
Investigation abstract
At 8:30 a.m. on July 1, 2021, Employee #1 was installing a new set of dies in press C2308. While attempting to set the die, the guide pins in the press bed holes loosened, causing the die to slip and strike the employee's head. To prevent a fall, the employee grabbed the press with his right hand. The top part of the die then slipped completely off the bottom die and fell onto his right hand. Emergency services transported the employee to the hospital, where he was admitted and treated for a laceration to the head and multiple fractures to the fingers on his right hand.
