Event description

Employee crushes foot while operating forklift

Investigation abstract

At 10:35 a.m. on May 5, 2021, Employee #1, a male employee, was operating a Raymond Forklift while depressing the operating pedal with his inside foot, which exposed his outside foot to the edge of the standing deck on the forklift. The employee's left foot extended outside the forklift's lines, exposing it to a caught-in-between hazard. As the employee backed the forklift between two racks, his left foot became trapped between the vertical support of the rack and the base of the forklift, causing serious injury. The supervisor on-site rendered first aid and transported the employee to the local hospital, where the employee was admitted for treatment of injuries to the left foot and lower leg.
