At approximately 12:00 p.m. on July 5, 2021, an employee was quarantined due to COVID-19 exposure. Employee tested positive for COVID-19 on July 12, 2021. The employer was contacted on July 24, 2021, with a notice that the employee died from severe symptoms from COVID-19.
