Investigation abstract
At 12:30 p.m. on June 3, 2021, Employee #1 was standing and talking with a coworker when he was struck by a powered industrial vehicle operated by a coworker. The impact crushed Employee #1 between the vehicle and a stationary guardrail. Employee #1 sustained fractures to both legs and was transported by ambulance to Riverside Medical Center in Kankakee, Illinois, where he was admitted and hospitalized for treatment.
