Investigation abstract

At 10:00 a.m. on July 26, 2021, an employee stood on a ladder placed on the third level of a scaffold. While working from the ladder, the ladder became unstable and fell off the scaffold. The employee fell approximately thirty feet and was killed by blunt force trauma.
