Event description

Employee amputates fingertip by cooling tower drive pulley

Investigation abstract

At approximately 1:00 p.m. on June 25, 2021, an employee was cleaning the roof of a facility and checking belts on a cooling tower that had been replaced the day before. While inspecting the belts, the cooling tower unexpectedly turned on, pulling the employee's finger into the drive pulley. The drive motor operated at 1775 RPM and was connected to pulleys measuring 7 ? inches and 31 ? inches in diameter, linked by four 140-inch belts running in parallel. The employee suffered an amputation of the tip of the finger at the base of the nail. The center heights of the pulleys were 51 inches and 7 ? feet above the roof. The employee was hospitalized for medical treatment.
