Investigation abstract
At 6:48 a.m. on June 2, 2021, Employee #1, a facilities maintenance mechanic at a golf equipment manufacturing warehouse, was working with a team to install LED lighting between industrial racks. While descending in an order picker (Crown #30SP36TT), Employee #1 placed his left hand on a horizontal steel lighting bar. As the order picker lowered, his hand was caught between the overhead protection cage and the steel bar, fracturing his left middle and ring fingers. Employee #1 was admitted to the hospital and treated for distal fractures, contusions, and bruises to the fingers. The incident was witnessed by two coworkers.
