Event description

Two Employees with COVID-19 infection are hospitalized

Investigation abstract

On July 5, 2021, two employees working as bus drivers working for a public from his spouse and doesn't recall receiving COVID-19 training. Employee #2 last worked on July 16, 2021. She began to experience symptoms on July 19, 2021, tested positive on July 20, 2021, and was hospitalized for 5 days around July 26, 2021. Employee #2 was not vaccinated and there are no barriers in the driver's seat to separate driver and passengers. Employee #2 reported no contact with coworkers during her job duties. The employer provided wipes, gloves and masks and the employee has access to hand washing facilities along her route. Employee #2 stated that she did not receive COVID-19 training and doesn't recall any rules to keep bus windows open during operations or open the doors at the end of the lines. Employee #2's source of exposure is unknown. The employer did transportation agency were reported hospitalized with COVID-19. Employee #1 last provide tools to protect employees from COVID-19. The employer did not document the COVID-19 hazards for bus drivers or document training for drivers. worked on June 25, 2021. He started to experience symptoms on June 25, 2021, one week after his spouse tested positive. His spouse did not develop symptoms. He was sent by ambulance to the emergency room where he tested positive and was hospitalized from July 5, 2021, to August 20, 2021. Employee #1 notified his employer on July 7, 2021, and started rehabilitation on August 20, 2021. Employee #1 was not vaccinated prior to contracting COVID-19 and reported he does not recall most of the details. Employee #1 believes he contracted COVID-19
