Investigation abstract

At 6:40 p.m. on July 31, 2021, Employee #1 was putting paper into a feed roller when the left pinky became caught in the rollers. While attempting to free the left hand with the right hand, the right hand also became caught. The employee sustained amputations to both hands and was hospitalized.
