Investigation abstract
At 11:30 p.m. on July 26, 2021, an employee was clearing paper plates from a dye press machine when they opened a gate/guard to remove debris by hand. The machine did not stop when the gate was opened, and the employee's left hand was caught, resulting in the amputation of four fingers. The employee was hospitalized for treatment.
