Investigation abstract
An employee was working at a nursing care or skilled nursing facility. She 2021. He followed this with an online "SER" on August 4, 2021. At the time, the employer indicated that there were no positive cases of COVID-19 at the center. This was not an OSHA-covered fatality. worked only on weekends. Her last day of work was Sunday, July 4, 2021. On Tuesday, July 6, 2021, she came in to test for COVID-19, as the facility tested regularly at least once a week. She tested positive. She was placed on quarantine at home. The employer claimed that he made attempts to communicate with the employee but could not reach her.
