Event description
Employee incurs partial finger amputation while using gluer
Investigation abstract
At 2:30 p.m. on August 4, 2021, Employee #1 was assigned as a helper on a Ward Flexo Gluer 18000, also known as the "125 Jumbo Press," to assist with processing corrugated sheets measuring 38 inches by 140 inches. While sitting on the machine during setup, Employee #1 leaned back, causing his hand to enter the machine under the guard. The coworker and machine operator immediately stopped the machine and, with the assistance of the supervisor and maintenance staff, extracted Employee #1. Paramedics transported him to the hospital, where he was treated for a partial amputation and crushing injuries to the fingers of his left hand.
