Investigation abstract

At 8:45 p.m. on May 5, 2021, an employee was operating an electric pallet jack p owered industrial vehicle (PIV) in a warehouse when he ran the PIV into product storages racks. The employee crushed his head and face between the PIV and the r acks and was killed.
