Investigation abstract

At 12:05 p.m. on August 3, 2021, at a wood treating facility, an employee of at the site on August 9, 2021. Proper guarding of the chain and sprocket could have prevented this accident. Fontana Wood Treating was engaged in processing wood planks on an even-ender conveyor system. While attempting to adjust the gap of the even-ender device to facilitate wood transport, the employee's left hand was caught by the chain and pulley mechanism. The machine lacked the necessary guarding to prevent contact with the chain and pulley, resulting in the amputation of multiple fingers, specifically the pinky, ring, and middle fingers of the left hand. The employer reported the injury to DOSH on August 4, 2021, in compliance with T8CCR �342. The employee was treated without hospitalization. The DOSH investigator arrived
