Investigation abstract
At 12:30 p.m. on August 9, 2021, an employee was operating and monitoring a swager machine when two pieces of rebar were fed into the gripper vise simultaneously, which is designed to hold only one piece at a time. As the employee reached to remove the second piece of rebar, his left ring and middle fingers were caught in the gripper vise, resulting in fingertip amputations. The employee was hospitalized for treatment.
